Abstract (eng): | The effects of limb length discrepancy on the physiology of lower limbs, hip and spine as well as on the quality of live have long attracted interest among surgeons. Leg length equalization can be carried out operatively by lengthening the shorter limb. In this technique, an artificial fracture is made to the femur or tibia after which the bone fragments are pulled apart with a device designed for this. Two main types of devices used for limb lengthening are external fixators and fully implantable devices also known as intramedullary nails. Complications related to external devices have made them less appealing despite the low cost of the device itself. As for implantable devices, higher cost of the device has slowed down their distribution, though the complication rate is found to be smaller. However, the device cost is just one part of the total costs of limb lengthening treatment of which the low complication rate could significantly reduce. The objective of this thesis is to find out if the methods using implantable devices are lower in total costs when compared with external devices. In addition to costs related to the normal treatment process, complication costs and costs caused by sick leave are taken into account. The hypothesis of this thesis is that lower complication rate and faster rehabilitation times with implantable devices produce lower total costs of the treatment. Altogether 40 patient reports were collected and read through in order to build a cost model for limb lengthening treatment. Total costs of the treatment as well as costs caused by complications were smaller with patients who used intramedullary nails. A bigger patient cohort is to be covered in the future so that the results can be made more reliable. The cost model built in this thesis is a good framework for this. |
Abstract (fin): | Alaraajojen pituuseron vaikutukset alaraajojen, lantion ja selkärangan fysiologiaan sekä yleisesti elämänlaatuun ovat pitkään olleet lisääntyvän mielenkiinnon kohteena. Pituuseroja voidaan hoitaa raajan pidennysleikkauksella, jossa lyhyemmän jalan reisi- tai sääriluuhun tehdään keinotekoinen murtuma ja luun päitä vedetään hitaasti ja hallitusti erilleen tarkoitukseen suunnitellulla pidennyslaitteella. Näiden pidennyslaitteiden päätyyppejä ovat kehon ulkoiset hoitolaitteet sekä täysin implantoitavat, luuydinnaulaukseen perustuvat laitteet. Kehon ulkoisiin hoitolaitteisiin liittyvät komplikaatiot ovat heikentäneet laitteen houkuttelevuutta sen edullisesta hankintahinnasta huolimatta. Luuydinnauloilla puolestaan kallis hankintahinta on hidastanut laitteiden yleistymistä, vaikka komplikaatiotaajuuden on todettu olevan alhaisempi. Laitteen hankintakustannus on kuitenkin vain osa raajanpidennyshoidon kokonaiskustannuksia, joita alhaisempi komplikaatiotaajuus voisi merkittävästi pienentää. Tämän diplomityön tavoitteena on selvittää, ovatko raajanpidennyshoidon täysin implantoitavia laitteita käyttävät hoitomenetelmät kokonaiskustannuksiltaan edullisempia kuin ulkoisia hoitolaitteita käyttävät menetelmät. Kokonaiskustannusten laskennassa otetaan huomioon hoitoprosessiin normaalisti kuuluvien kustannusten lisäksi komplikaatioista aiheutuvat kustannukset sekä potilaiden työkyvyttömyydestä aiheutuvat kustannukset. Tutkimushypoteesina on luuydinnaulojen alhaisempi komplikaatiotaajuus ja nopeampi kuntoutuminen, joka tuottaa alhaisemmat hoidon kokonaiskustannukset. Diplomityötä varten kerättiin 40 raajanpidennyspotilaan potilasasiakirja-aineisto, jonka perusteella muodostettiin malli kokonaiskustannusten laskemiseksi. Tutkitun potilasaineiston luuydinnaulapotilailla kokonaiskustannukset olivat pienemmät kuin ulkoisilla hoitolaitteilla hoidetuilla potilailla. Myös komplikaatiokustannusten osuus kokonaiskustannuksista oli luuydinnaulapotilailla pienempi. Jatkossa tutkimukseen tulee sisällyttää suurempi potilasaineisto, jotta tuloksista saadaan luotettavampia. Tässä työssä muodostettua kustannusmallia voidaan soveltaa myös suurempaan potilasaineistoon. |
