Abstract:In the NHS (National Health Service) a combination of under-funding and inefficiency have conspired to the detriment of the patient. Under the reforms district health authorities will shop around to buy the best value services for their local population on the basis of contracts negotiated with the hospitals. These, in their turn, will be encouraged to opt out of direct health authority control and become self-governing trusts. With a cash-limited budget, a balance will be struck between the need to provide quality services to local people and the fact that, as a teaching hospital with highly developed skills, St James' will attract more complex (and more expensive) work from elsewhere.
