Abstract:The accidental release of methylisocianate in Bhopal may prove to be the worst industrial accident to date in terms of lives lost. A detailed analysis clearly shows that safety management was poor and ad hoc at the plant level. The types of errors at corporate level can be classified into three categories, namely hard error relating to technology, hardware, instrumentation and structural defects, human error relating to judgement and intuition, and system error relating to procedures and institutionalization of safety. To conclude this incidence makes it necessary for developing countries to reexamine their policies towards toxic substance handling.
