Abstract (eng): | Ageing populations create a need for increasing the efficiency of primary health care production. Demand management has an important role in creating efficient health care systems. In this Master's Thesis, demand management in primary care was analyzed. The new model for the operations of a Finnish health centre and its emergency clinic was developed. The empirical part of this research is based on a case study in a large Finnish health centre which produces primary health care services for 42 000 inhabitants. Numeric data for research was collected from the health centre's CRM-database and a survey. In the survey the number and nature of client contacts were recorded during a five-week period, the total amount of contacts being nearly 11 000. According to the study, the availability of care varied a lot depending on the residential area and the type of appointment. The time of reservation had a significant influence on the availability of urgent care. During the five-week period, the availability of care was 57 % in urgent cases and 91 % in non-urgent cases. The most significant operative problems at the health centre operations were associated with the personal general practitioner system, in which a general practitioner is allocated for the every inhabitant. There are 17 general practitioners in the case health centre. The standard deviation for daily demand for single physician appointments was observed to be large to the extent, that forecasting was impossible. However, demand for the entire health center was observed to be highly predictable. For example, demand for urgent care was practically stable. The inflexibility of resources led to poor availability of care, although there were empty appointments continuously. The research observed prominent improvement potential in demand forecasting, if the forecasted unit consists of several physicians instead of one physician. This also applies, if weekly demand was studied instead of daily demand. According to the study, health centre operations can be improved with small changes. At the same time customer service can be improved. The primary care system should be developed towards a system, in which only patients who use a lot of health services get specified general practitioner. |
Abstract (fin): | Väestön ikääntymisestä johtuva terveyspalveluiden kysynnän kasvu aiheuttaa terveys-palveluiden tuotantoon tehostamistarpeita. Kysynnän hallinta on tärkeässä asemassa luotaessa tehokkaasti toimivaa terveydenhuoltojärjestelmää. Tässä diplomityössä selvitettiin miten perusterveydenhuollon kysyntää voidaan hallita. Samalla tutkimuksen kohteena olevan terveysaseman toimintaa kehitettiin luomalla sille muun muassa uusi päivystystoiminnan malli. Tutkimus suoritettiin tapaustutkimuksena suurella suomalaisella terveysasemalla, joka tuottaa perusterveydenhuollon palvelut noin 42 000 asukkaalle. Tutkittava aineisto koostui potilastieto-järjestelmästä saatavasta tiedosta, jota täydennettiin ajanvarauspisteissä suoritetulla mittauksella. Ajanvarauspisteissä kirjattiin viiden viikon ajalta asiakaskontaktien määrä ja sisältö, kontakteja kertyi yhteensä lähes 11 000. Hoidon saatavuuden havaittiin vaihtelevan merkittävästi potilaan asuinalueesta ja vastaanottotyypistä riippuen. Lisäksi kiireellistä hoitoa vaativissa tapauksissa ajanvaraushetki vaikutti suuresti vastaanottoajan saatavuuteen. Vastaanottoaikojen saatavuus oli tarkastelujakson aikana kiireellisissä tapauksissa 57 prosenttia ja ei-kiireellisissä tapauksissa 91 prosenttia. Merkittävimmäksi terveysaseman toimintaa hankaloittavaksi tekijäksi havaittiin omalääkäri-järjestelmä, jossa asukkaille nimetään henkilökohtainen lääkäri. Tutkitulla terveysasemalla toimi 17 omalääkäriä. Yksittäisen lääkärin kokeman vuorokausikysynnän vaihtelu havaittiin niin suureksi, että sitä oli mahdotonta ennakoida. Kuitenkin koko terveysaseman tasolla vastaanottojen kysyntä oli hyvin ennakoitavissa. Esimerkiksi akuuttihoidon kysyntä oli käytännössä tasaista. Resurssien joustamattomuus johti hoidon huonoon saatavuuteen, vaikka samalla vastaanottoaikoja jäi jatkuvasti täyttämättä. Kysynnän ennustettavuuden havaittiin paranevan huomattavasti, kun tarkasteltiin yhden lääkärin sijasta useamman lääkärin ryhmää. Samoin tapahtui, kun vuorokauden kysynnän sijasta tarkastelun kohteena oli koko viikon kysyntä. Tutkimuksessa havaittiin, että terveysaseman toimintaa voidaan kehittää pienillä muutoksilla. Samalla asiakaspalvelua voidaan parantaa. Perusterveydenhuoltoa tulisi kehittää kohti järjestelmää, jossa ainoastaan paljon palveluita käyttäville potilaille nimettäisiin henkilökohtainen lääkäri. |
