Abstract (eng): | The subject of this master's thesis is to present the design process of a telephone service approach for occupational healthcare directed to handle short sick-leaves. This research has been carried out by the initiative of Doctagon Ltd., which has previous experience from telephone services in different contexts of healthcare. Sick-leaves are unavoidable, but they cause challenges for the working-life and for society. For example, in the Finnish context employers have to pay sick-leave compensation to the sick employee from the first day of illness as well as for the following nine sick-days, in addition to the other arrangements that the sick-leave might cause. On the other hand, in Finland, employers are allowed to demand a doctor's certificate with diagnosis, if the employee wishes to be paid for the sick-days. However, it is noteworthy that the doctor's certificate is an estimation done at the beginning of the sick-leave period, and its written diagnosis and length are predictions for the forthcoming days. A large share of short sick-leaves is caused by common physical conditions whose treatment only requires rest and home-care. Hence, in the case of short-sick leaves, the patient would not often need medical care, but visits the doctor only because their employer requires a doctor's certificate for short sick-leaves. It is also often hard to predict the future condition of the patient and this might lead to a situation where slightly too long sick-leaves are written so that the patient would not have to come to the doctor's practice just to get another certificate. The service process designed in this master's thesis has taken into consideration these challenges. In the service designed, a nurse contacts the patient by telephone every day and they then together estimate the current situation regarding the patient's ability to work as well as the amount of time that may still be required to recover enough to get back to work. This way it is possible, in a dynamic way, to daily follow the changes in the patient's health situation and the required length of recovery, and also to avoid unnecessary visits to the doctor's practice in cases in which the primary reason for the visit would be to get a doctor's certificate. First presented in this thesis is material from the literature and specialist interviews regarding the patient's ability to work, the Finnish occupational healthcare system, and the current national situation of sick-leaves as well as a summary of the relevant theory of designing service processes. In the empirical section, the background of the new service process design and the actual service blueprints are presented. Lastly, experiences of the service process during its first month in use, and results from questionnaires posed to managers and employees in the pilot company concerning their presuppositions towards the service are gathered. The most significant result drawn from the experiences and the questionnaires is that no need for significant changes in the service process design is seen at this point. |
Abstract (fin): | Tässä diplomityössä esitetään suunnitteluprosessi ja sen vaiheet lyhytaikaisten sairauspoissaolojen etähallintaan tarkoitettuun telepalveluun. Tutkimuksen toimeksiantajana on Doctagon Oy, jolla on aiempaa kokemusta puhelimitse toteutettavasta sairaanhoidosta eri terveydenhuollon konteksteissa. Sairauspoissaoloja ei voida välttää, mutta ne aiheuttavat haasteita työelämälle ja yhteiskunnalle. Esimerkiksi Suomessa työnantajalla on velvollisuus maksaa sairauspoissaolosta korvaus työntekijälle sairastumispäivältä ja sitä seuraavilta yhdeksältä sairauspäivältä mahdollisten muiden sairauspoissaolon takia tehtävien järjestelyiden lisäksi. Toisaalta Suomessa työnantajat saavat vaatia työntekijöiltään diagnoosillisen lääkärintodistuksen ensimmäisestä sairauspäivästä lähtien palkanmaksua varten. Suuri osa lyhyistä sairauspoissaoloista johtuu tavallisista taudeista, jotka paranevat itsekseen kotihoidossa, ja siten potilas saattaa usein käydä lääkärin vastaanotolla vain saadakseen työnantajansa haluaman sairauspoissaolotodistuksen. On myös huomionarvoista, että lääkärintodistukseen kirjoitettu diagnoosi ja kesto ovat ennusteita potilaan terveydentilan kehityksestä, jonka takia toisinaan kirjoitettu sairausloman kesto saattaa olla varmuuden vuoksi tietyn pituinen, jotta potilaan ei tarvitsisi tulla vastaanotolle uudestaan turhaan vain uuden todistuksen hakemista varten. Diplomityössä suunnitellussa palveluprosessissa on lähdetty edellä esitetyistä haasteista. Suunnitellussa palvelussa työkyvyn arvio tehdään päivittäin hoitajan soittaessa potilaalle, jolloin he yhdessä tekevät työkyvyn arvion ja määrittävät paranemiseen vielä tarvittavan sairauspoissaolon keston. Näin voidaan huomioida päivittäin yksilöllinen terveydentilan kehitys sairauspoissaolon aikana ja välttää myös turhia vastaanottokäyntejä sellaisissa tilanteissa, joissa sairauspoissaolotodistuksen noutaminen olisi vastaanotolla käynnin pääasiallinen syy. Tutkimuksessa käsitellään aluksi kirjallisuuskatsauksen ja asiantuntijahaastatteluissa kerätyn aineiston avulla työkyvyn käsitteistöä, suomalaista työterveysjärjestelmää ja sairauspoissaolojen nykytilaa, sekä tiivistetään palveluiden suunnittelulle oleellisia teoreettisia näkökohtia. Empiirisessä osiossa esitetään palveluprosessia varten tehdyt suunnitelmat ja niiden taustatyö. Viimeiseksi tutkimuksessa esitellään suunnitellun palvelun ensimmäisen kuukauden käyttökokemukset sekä palvelun pilottikohteen esimiestason ja työntekijöiden ennakko-odotukset ja asenteet palvelua kohtaan. Ensimmäisen kuukauden käyttökokemusten ja ennakkokyselyiden merkittävimpänä tuloksena saatiin, että tarvetta muutoksille tässä tutkimuksessa suunniteltuun prosessiin ei havaittu tässä vaiheessa. |
