Abstract (eng): | Several ophthalmologic therapies are based either on rapid heating of tissues to high temperatures or maintaining the controlled temperature elevation of tissues for longer period. For safe therapy methods noninvasive thermal monitoring of tissues are required. However, the noninvasive thermal monitoring of living subjects is scientifically demanding problem: The tissue can be located far from the surface in which case traditional thermal imaging techniques are not accurate enough. Furthermore, other complications for reliable temperature monitoring methods are that the tissue layer under heat therapy is thin and its temperature is close to its surroundings so that resolving the thermal emission from the heated tissue layer is difficult. This survey focuses on the problems that are related to thermal monitoring of the retinal pigment epithelium (RPE) under heat therapy. The RPE is a 10 micrometer thick layer located at the bottom of the retina, and it is highly absorbing tissue due to its melanin content. To describe the heat flow at eye tissues during RPE heat therapy, this thesis consist of introducing three related topics: 1) optical properties of eye tissues in the optical axis, 2) heat transfer models, and 3) temperature monitoring methods. The first part of this thesis is completely based on the available literature. In the second part the radiative transfer theory is shortly presented. In addition to theory of radiative transfer, models for calculating the equilibrium temperature within a tissue, that account of heating and heat convection due to blood circulation, are introduced. The last part of this work presents promising spectroscopic methods that have been used to the retinal imaging. Furthermore, the possibility to apply the spectroscopic methods for noninvasive temperature monitoring is discussed. |
Abstract (fin): | Tämän työn tarkoituksena on selvittää, miten kudoksen lämpötila voidaan määrittää epäsuorasti. Tällainen tilanne on ilmeinen monissa lääketieteellisissä lämpöhoitomenetelmissä. Perinteisen infrapunakuvantamiseen perustuvan menetelmän heikkous on, että sen avulla pystytään määrittämään vain lämpötila pinnan läheisyydessä. Tarkoituksena tässä työssä on tutkia sellaisen ohuen kudoksen lämpötilan määräämistä, jota lämmitetään ja jonka lämpötila nousee vain hieman ympäristöään korkeammaksi. Erityisesti työssä käsitellään lämpöhoidon alaisena olevan verkkokalvon pigmenttiepiteelin lämpötilan määräämiseen liittyviä ongelmia. Verkkokalvon pigmenttiepiteeli on 10 mikrometriä ohut verkkokalvon pohjalla sijaitseva kudos, joka sisältää voimakkaasti absorboivaa melaniinia. Tämä tutkielma koostuu kolmesta osasta: 1) silmän kudosten optisista ominaisuuksista, 2) lämmönkuljetusmalleista ja 3) lämpötilan määrityksessä käytettävistä menetelmistä. Työn ensimmäinen osa perustuu kirjallisuuslähteisiin. Tutkielman toinen osa käsittelee säteilyn kulun teoriaa. Lisäksi työn toisessa osassa esitellään tasapainolämpötilan määrittämiseen soveltuvia malleja, jotka huomioivat mm. lämmityssignaalin vaikutuksen sekä verenkierron aiheuttaman lämmön konvektion. Työn viimeisessä osassa käsitellään sellaisia spektroskopiaan perustuvia menetelmiä, joita on käytetty verkkokalvon rakenteen tutkimisessa ja joita on sovellettu lämpötilan määrittämisessä. Tutkielmassa esitetyt spektroskooppiset lämpötilan mittausmenetelmät perustuvat veden spektrin siirtymään lämpötilan muuttuessa. Työn viimeisessä osassa esiteltyjen menetelmien jatkokehitys saattaa mahdollistaa lämpötilan mittaamisen termoterapian aikana. |
